Cardiology 32 years experience. 2019-11-15 The commonest CAA is a separate origin of the LAD and LCX, with an incidence of 0.41%, followed by LCX arising from the RCA, with an incidence of 0.37% [2, 9, 24-26]. Although congenital coronary artery anomalies are relatively uncommon, they are the second most common cause of sudden cardiac death (SCD) among young athletes. Origin of the RCA , LAD artery , or LCX artery from the pulmonary artery has also been reported, in variants of this syndrome. Origin of the RCA from the pulmonary artery in a 38-year-old man with chest pain and anomalous anatomy at cardiac catheterization.

When the RCA gives off PDA, the anatomy is referred to as right-dominant system (if the Lcx gives off PDA, it is referred to as left-dominant system). In patients with right-dominance the RCA supplies the atrioventricular (AV) node. In 60% of individuals the right coronary artery gives off branches to the sinoatrial (SA) node. The LAD and LCX are seen in their respective locations, but they receive blood from the RCA. This is an extremely rare condition and differs from a single RCA because some of the branches fill retrograde through the RCA. 2 The collateral circulation from the right to the left coronary system is usually not sufficient so almost all patients Left coronary artery (LMCA): The left coronary artery has a short common stem (and is hence often referred to as the left main coronary artery), that bifurcates into the left circumflex artery (LCx), which courses over the left atrioventricular groove, and the left anterior descending artery (LAD), which passes towards the apex in the anterior interventricular groove. The origin of LCX from the main RCA is a common anomaly.

One of the anatomical variants of coronary artery origin comprises the left circumflex coronary artery (LCx) and left anterior descending coronary artery (LAD) arising separately from the left coronary sinus.
